Covid-19 vaccination in Nigeria

Covid-19 vaccination in Nigeria is set to begin. According to a tweet by National Primary Health Care Development Agency (NPHCDA), which was re-tweeted by Chikwe Ihekweazu of NCDC.

The Federal Ministry of Health Covid-19 Vaccination Registration portal is opened for electronic registration.

It captures your data and schedules you for the covid-19 Vaccination. Registration at the portal will make your vaccination easier and faster.

During the registration, applicant will fill his full name, contact phone number, email address, date of birth (or enter your approximate age), gender, type of ID, state of residence, LGA of Residence.

Select your preferred time slot i.e. morning or afternoon. Upload your passport photo.

Then you will answer yes or no to the following questions:

  1. Do you work in the Health Sector?
  2. Any known medical condition?
  3. Any History of Allergies?

Once you are done filling the form, you will then hit the submit button.

After submitting the online form, they will display the below message:
